No Longer Primary Care Provider as of April 2023
Dear Patients and Families,
I am writing to let you know that my last day as a primary care provider at Family Health Center will be on July 21, 2023. While I will continue to work as a physician in Worcester, I will be doing community-based work and will not continue to have a primary care panel in the way that I have up until now.
It has been a sincere honor to be your primary care provider over the past many years, to get to know you and your families, and to be a part of your lives. I am grateful for the trust and confidence that you have shown me, and the stories and experiences we have shared. Our interactions have shaped me as a person and as a physician, and will stay with me as I enter the next part of my life.
Even though I will not be a primary care provider at Family Health Center, you are welcome to continue receiving care here, and will receive complete medical care from Team 4 until such a time as you are assigned to a new primary care provider. Please contact the Health Center at 508-860-7700 to schedule any appointments, for medication refills, or with medical concerns.
Please note that your medical information will remain with Family Health Center as I move on. If you wish to access or have copies of your records, please contact the Medical Records Department of the Health Center.
