Changing Spaces: Enhancing Your Care Experience at Family Health Center

At Family Health Center, we’re committed to delivering the highest quality care in the most efficient and compassionate way possible. To better serve our patients, we’ve launched a comprehensive initiative to improve how care is delivered across our Health Center. 

As part of this effort, several teams are relocating within the Health Center. Some will be integrated directly into the care teams they support, while others will move to new floors or suites. These changes are designed to make care more seamless, accessible, and coordinated. 

Why We’re Making These Changes 

We’re taking an interdisciplinary approach—bringing together clinical and operational leaders—to address some of the most important challenges in healthcare today. Our focus is on four key areas: 

  • Better Use of Space: By analyzing how rooms are currently used and how patients move through the Health Center, we’re identifying ways to reduce bottlenecks and improve scheduling. 
  • Primary Care Provider (PCP) Continuity: Maintaining strong relationships between patients and their PCPs is essential for satisfaction and health outcomes. We are developing models that allow urgent needs to be met while preserving continuity for routine care. 
  • Improved Access to Appointments: We’re working to make it easier to get same-day or timely appointments by adjusting provider schedules, extending service hours, and enhancing our telehealth offerings. 
  • Integrated Care Teams: We’re bringing together services like primary care, interpretation, care management, community health workers, and behavioral health case management—all in one place. This team-based approach ensures that every aspect of a patient’s health is addressed in a coordinated and compassionate way. 

Recent Team Relocations 

To support this new model, several teams have already moved: 

  • Office-Based Addiction Team → Now located on the 1st Floor 
  • Perinatal/Obstetrics Team → Now part of Team 4 
  • Interpreter Services → Now integrated directly into care teams across the Health Center 

More relocations will be announced in the coming weeks—stay tuned!
