Looking for resources related to receiving primary care services? 

We’ll be updating the information regarding primary care and care team resources regularly. See below:

• If your primary care provider has left, you will be assigned to a Care Team made up of providers and support staff to care for your health needs while you await a new primary care provider.

• The assignment to Care Team will occur over the next month and should be complete by January 2023.

• If you need to contact your assigned Care Team, dial 1-508-860-7700, then follow the phone prompts to be connected to your team.

• We will make every effort for you to be seen on your assigned Care Team when you need care.

• As we expand our primary care capacity, we will be assigning you to a new primary care provider.

• We thank you for your patience during this time of transition.
